People killed in mining accidents over the past year are deplorable said The Queensland Mines Inspectorate (QMI).
This financial year has been one of the most horrible for losses on Queensland mine sites.
Four people have died in work accidents, which are the most terrible figure since 1996-'97, when 10 people were killed.
The industry is worsening to give health and safety enough precedence and that the deaths are unacceptable says the annual report of QMI.
The report says site supervisory should make sure all workers, as well as contractors, put health and safety first and that safety documents are taken sincerely.
It says more desires to be done to make certain safety actions are related to work situations and emphasize possible hazards.

On Saturday, when 131 miners were working underground, 30 miners were killed when colliery gas burst in Tonghua coal mine in Qijiang County in Chongqing municipality.
The official Xinhua news agency reported that 101 miners were rescued. 59 miners were injured of the rescued, together with four in serious condition, it said.
The mine accident is happen by an illegal practice which violated the mining rules, said Luo Lin, chief of the State Administration of Work Safety.
Luo said the Cabinet will set up a special investigation team to check out the coal mine explosion.
An extreme amount of explosives directly triggered the accident, said Zhao Tiechui, head of the State Administration of Coal Mine Safety.
The mine's managers didn't abandon workers in time, said Zhao. Police have held three people including the coal mine's owner, chief engineer and project manager in link with the accident.
China's coal mines are thought the world's most dangerous. Official figures illustrate that over 3,200 miners killed last year but the actual figures are much more than that as several such accidents are covered up.
